Stable Booking Form

Please complete this Stable Booking Form for all new stable bookings. To complete this form, you will need to specify the type of membership required and the anticipated length of stay. The form outlines the terms and conditions for private clients.

In addition to this signed application form you will need to have:

• Submitted a signed Code of Conduct and Regulations declaration
• submitted a completed Declaration of Animal Health
• Provided a Tetanus and Strangles Vaccination Certificate (issued by a veterinary surgeon in the past 12 months)
• Provided evidence of appropriate insurances for public liability and third party property (either copies of current insurances or evidence of current EA membership)
