Applicants for ES Marks Athletics bookings are required to please fill in our interactive form. In conjunction with this form please review our Fees and Charges, Terms and Conditions and Regulations (see below for links and more details).
The form is an interactive PDF which allows you to type the applicable information directly into the form. Prior to using this form you must have Adobe Reader version 8.2 or above installed on your PC or Mac. This form will only function correctly when opened in Adobe Reader. Adobe Reader can be freely downloaded from the Adobe Australia website.
Please open and save the form regularly to your hard drive. It can then be emailed as an attachment together with any other documentation as outlined in the form.

Equipment hire
Centennial Parklands does not hire athletics equipment. Please go to our editable PDF application form for school carnivals booking or contact Athletics NSW on (02) 9746 1122 for equipment once your carnival has been confirmed in writing and full payment and a copy of the public liability insurance has been received.
