Make a formal request
To make a formal Right to Information request please take the following steps:
- Identify the information or document(s) you would like to see;
- Identify which agency or organisation is likely to keep that information. If you are unsure about the agency concerned, contact the agency's Right to Information officer or the Office of the Information Commissioner on free call 1800 INFOCOM (1800 463 626) or at its website: www.oic.nsw.gov.au
- Write your request on our Rights to Information application form (PDF, 36kb). If you cannot download the PDF please phone the Trust on (02) 9339 6699
- Submit your application form by post to

Address: Locked Bag 15, Paddington NSW 2021 - Provide as much information as possible about the document(s) you wish to see, such as file reference numbers; and
- Enclose the $30 fee with your application. If you are a pensioner, a representative of a not for profit organisation, a full time student or have financial hardship, the application fee is reduced to $15 (photocopy or proof of Centrelink benefit or Health Benefit Card to be provided with application).
